Chronic diseases are becoming a major concern worldwide, affecting about 25% of adults and putting a strain on individuals, families, and healthcare systems. The “Smart Healthcare” era has introduced advanced technologies to manage these conditions better. By integrating AI, blockchain, and wearable technology, we can create a patient-centered approach to managing chronic diseases, moving away from a hospital-centered model. This paper presents a conceptual framework based on these technologies and explores their application in chronic disease management.
