Patient-Led Transmural Health Pathway Design: Hospital-to-Home Transitions

This study explores the hospital-to-home transition for patients, emphasising the role of allied healthcare. Conducted through interviews with 19 patients, it identifies three key themes: positive experiences with allied healthcare support, the importance of patient and family involvement in discharge planning, and challenges in processing overwhelming information during discharge. The findings highlight the significance of continuous collaboration among healthcare professionals and personalised support for patients and families to enhance the quality of transitional care.
